What is the First Responder Smart Card program?
Action Partnership, in cooperation with local First Responders (Police,
Fire and EMS,) have
worked together to implement the First Responder Smart Card Program
(FRSCP)
in the Verde Valley. This program offers a registration process,
alerting First Responders that there is a Smart Card on site and an individual
with special needs may be at the registered address.
The Smart Card is a form that you fill out, displayed at your residence and in
your vehicle, that will provides critical information to first responders
concerning an individual with special needs. FRSCP includes training for
First Responders on special needs, and how to better recognize and respond to an
individual with special needs. This program also provides training for parents
and providers on when to call first responders and what happens once the call is
made.
The Verde Valley area FRSCP covers
Cottonwood, Clarkdale, Jerome, Sedona, Camp Verde, Rimrock, Cornville and the
unincorporated areas of Eastern Yavapai County. For clarification on

Registration is Simple
If you are registering a residence in the
Verde Valley area, please fill out the form below with the residence
address and the contact information. A packet will be mailed to you in a few
weeks, please make sure to follow the enclosed instructions, complete your smart
cards, and display your sticker.
