CHINO
AREA PARTNERSHIP PROJECTS
Tri-City Medical Home
Our Mission for the Tri-City Medical Home project is to improve the
quality of pediatric healthcare and related services for children with
special health care needs. The medical home is not a building, home or
hospital. Our approach is to build upon the existing infrastructure in
our community to develop a network that includes parents, primary care
physicians, specialty care providers, care coordinators and community
members.
Our Vision is for children with special health care needs, in the
Tri-City area, to receive comprehensive, high quality and cost effective
health care services from provider’s that families trust. We are
currently focusing on Welcome to Holland® project which is a resource
bag with the theme based on the poem written by Emily Pearl Kingsley.
We meet before the partnership meeting on the first Thursday of each
month from 9:30-11:00 in the Chino Valley Unified School District Board
Room. Anyone interested in getting more information on the Tri-City
Medical Home project or attending our meeting please call our Community

S.E.L.F. Club
Special Extraordinary Library Fun is a seven week summer literacy
program for children with special needs and their siblings. This program
partners with the Chino Valley Public Library, Chino Valley Recreation
Department, Chino Valley Unified School District: Transportation
Department and Food Services. We meet once a week during the summer for
2 hours. The program consists of Parks and Recreation staff providing
activities for our kids, the library provides a craft and/or special
guest, and a “Free” bagged lunch is provided by the Chino Valley School
Food Services for children. Transportation is provided for those in
Chino Valley / Paulden area.
SWift® Resources is a partnership between Southwest Institute and
Community Action Teams across Arizona dedicated to individuals with
special needs.
www.swifamilies.com is a free website that connects people across Arizona bringing information together for:
- Children, Youth and Adults
with Special Needs and their Families
- Professionals, Providers and
Community Members
The goal of SWIft® Resources of
Arizona is to connect people with one another to share expertise,
lend support and ideas or provide a service.
Resources include:
- People with unique skills,
talent and knowledge
- Community Organizations
- Service Providers
(Northern Arizona Behavioral Health team)
The mission of the Family Leadership Network is to ensure a culture of
recovery and empowerment for all families within the behavioral health
system, our communities, and society as a whole through education,
reliability, and advocacy
We currently meet the last Tuesday of the month from 10 – 2 pm in
Flagstaff. If you are unable to meet in person telemed may be available
