CHINO AREA PARTNERSHIP PROJECTS

Tri-City Medical Home

Our Mission for the Tri-City Medical Home project is to improve the quality of pediatric healthcare and related services for children with special health care needs. The medical home is not a building, home or hospital. Our approach is to build upon the existing infrastructure in our community to develop a network that includes parents, primary care physicians, specialty care providers, care coordinators and community members.

Our Vision is for children with special health care needs, in the Tri-City area, to receive comprehensive, high quality and cost effective health care services from provider’s that families trust. We are currently focusing on Welcome to Holland® project which is a resource bag with the theme based on the poem written by Emily Pearl Kingsley.

We meet before the partnership meeting on the first Thursday of each month from 9:30-11:00 in the Chino Valley Unified School District Board Room. Anyone interested in getting more information on the Tri-City Medical Home project or attending our meeting please call our Community Development Leaders @ 636-9623 / 636-4815.
