OUR MISSION
To substantially
improve the quality of life, resources and opportunities for people with
special health care needs.

WHO WE ARE:
-
Flagstaff Community Partnership (FCP)
is a parent-led community action team, composed of families,
professionals, service providers and Flagstaff community members.
-
FCP is a unique, coordinated and
diverse cross-section of families and professionals who are
committed to creating and supporting positive change in our
community.
-
FCP began as the Community Action
Council (CAC) with parent leaders in 1999. The CAC had been around
since the 1980s but was not parent led. FCP’s mission is to
substantially improve the quality of life, resources and
opportunities for people with special health care needs.
WHAT WE DO:
-
We come together on
a monthly basis to share information and new resources in our
community to connect families with professionals, and professionals
with professionals.
- We provide trainings and workshops
to better equip families and professionals with up to date
information to strengthen families and enhance our community.
- FCP is a community venue for
linking diverse agencies, organizations and families to work
together to help build a cohesive, supportive partnership in our
community.
President's Message
“As President of the FCP Board of
Directors, I am honored to have such an important role in helping
the community of Flagstaff be a better place for people with
disabilities and their families. I am a lifelong resident of
Flagstaff and have devoted my life to being a well informed, well
spoken advocate for people with disabilities and their families.
FCP has provided me with the opportunity to network with community
and statewide agencies to provide various resources to people with
disabilities and their families so that they may have their voices
heard therefore gaining greater independence and self sufficiency.
I not only work for a living to improve the lives of people with
disabilities and their families, but, I am a young man with
disabilities myself who believes in increasing knowledge of the
Americans with Disabilities Act and the valuable resources that are
available to all of us throughout our community, state and nation.
My work is literally my life because I live with disabilities 24
hours a day 7 days a week. FCP will enrich my life for years to
come.”

Signs of Our Impact
-
Approached
by Grand Canyon Youth to help put
together a river trip in the summer of 2007 for individuals with
special needs
-
Parent
leaders asked to sit on hiring panels and program review committees
-
Parent
Leaders asked to present FCP Leadership and Community Development to
various agencies/organizations, directors
-
Flagstaff
area First Responders are committed and invested in bringing this
program to our area
-
Northern
Arizona Chapter of the Autism Society of America came to FCP asking
us to partner with them for the May 2007 Autism Conference
Visit our photo gallery to see photos of some youth teaching nursing students at NAU on how to interact with
people with special needs. What a great experience for the youth to become
'teachers' and the opportunity to increase the education of nursing students!

FCP Team Projects
These are collaborations, workshops, public events and trainings that
FCP has sponsored, participated and received support from the community.
- November 2005- sponsored Arizona
Governor’s Council on DD Public forums
- February 2006- Legislative
training with Representatives Tom O’Halleran and Ann Kirkpatrick
- March 2006- Partnered with NAMI
and The Guidance Center for NAMI Walks 2006. We did raffles, fund
raising and public awareness
- March 2006- Posters for People
with Mental Illness Enrich Our Lives were printed and distributed
- April 2006- First Responders Smart
Card Program was presented and is in the process of being introduced
into the Flagstaff community
- April 2006- Children’s
Rehabilitative Services (CRS) Fun Day
- April 2006- Transition Fair
- May 2006- Co-sponsored a special
education mini-conference with Children’s Health Center, CRS, NAU
and IHD (presented from Arizona Center for Disability Law)
- May 2006- Valuing Children’s and
Teens Abilities: Using 40 Assets to see Beyond Disabilities
- June 2006- Peaks Weightlifting
Club Open House
- July 2006- Helped to send a
Flagstaff family to the 11th Annual Conference for the National
Organization for Albinism and Hypopigmentation (NOAH). They will
present the information they received to the team in October 2006
- August 2006- Successfully received
DHS/OCSHCN support contract
- September 2006- FCP and CRS
collaborated to bring a region-wide information distribution site to
CRS. This site will serve as a centralized resource for families and
professionals in Northern Arizona and is to be maintained by the
parent leaders
- October 2006- On the County Board
of Supervisors agenda
